Registration Forms
Current Registration for One Spark Academy is for Imaginession, an enrichment-only session that begins Tuesday, May 29 for seven days. All classes are first come, first served. Space is limited, and confirmed with payment.
Please download the 2-page OSA Imaginession Registration. Please read the directions for registration, which are on the form, or here.
Your child must also read, print out, and sign the Participant Agreement.
If not registering in person, these forms must all be sent to our business mailing address prior to your student beginning his/her courses:
